Positive Behaviour Support Plans

A Positive Behaviour Support Plan (PBSP) is a written, evidence-based document developed to help improve the quality of life of a person with a disability and reduce behaviours of concern. It explains why behaviours may be occurring and outlines practical, tailored strategies that build skills, increase independence and support safer, more positive everyday experiences. PBSPs focus on understanding and support, and are delivered as part of positive behaviour support services by our skilled Positive Behaviour Support Practitioners.

At Outcomes Connect, we deliver evidence-based Positive Behaviour Support Plans that help people experiencing behaviours of concern feel safer, more supported and better understood. We recognise that behaviours can be complex and challenging for individuals, as well as for families, carers and support teams.

Our approach focuses on understanding the underlying reasons behind behaviour and developing practical, personalised strategies that reduce risk, build skills and improve quality of life. Through consistent, in-person support guided by a Positive Behaviour Support Plan, we work alongside participants and their support team to create plans that are realistic, ethical and designed for long-term positive change.

Interim Behaviour Support Plans

An Interim PBSP is a short-term plan developed when there is an immediate risk of harm or when restrictive practices are being used. It focuses on keeping people safe while a full assessment is completed. Interim plans outline temporary strategies and must be reviewed and replaced by a comprehensive plan within the required timeframes.

Comprehensive Behaviour Support Plans

A Comprehensive PBSP is a detailed, long-term plan developed after a full assessment of behaviour and underlying needs. It includes proactive positive behaviour support strategies, skill-building and clear guidance for families and their support teams.

A Behaviour Support Plan can only be written by a suitably qualified Behaviour Support Practitioner who meets the NDIS Quality and Safeguards Commission requirements. These practitioners are equipped with the training and experience needed to assess behaviour, use evidence-based approaches and develop ethical, person-centred plans that support safety and wellbeing.

Our plans are developed by suitably qualified Behaviour Support Practitioners who meet the requirements under the NDIS Quality and Safeguards Commission. Our practitioners bring relevant training, practical experience and ongoing professional development.

We use evidence-based approaches that focus on understanding the underlying causes of behaviour rather than managing symptoms. This includes gathering information from families, carers and support providers, getting to know the individual, what matters to them and how they experience their environment, and detailed assessment of behaviours of concern to understand when, where and why they occur. Each plan is tailored to the individual, prioritising dignity, choice and practical strategies that build skills and improve everyday quality of life.

Outcomes Connect is committed to reducing the use of restrictive practices wherever possible. Our plans focus on proactive strategies, environmental supports and skill development to promote safety while protecting individual rights.

If you’re ready to work with an NDIS provider who puts people first and focuses on meaningful, lasting change, we’re here to help.
