About Outcomes Connect Australia
Our Story
Delivering quality Positive Behaviour Support to all
Outcomes Connect Australia began in Victoria in 2018, working alongside people, families and support teams who needed NDIS Support that works in real life.
Our name stemmed from our genuine belief: that better outcomes are delivered through true connection. Our goal is to deliver this consistently across Australia, so that everyone gets the support they need.
From the start, the focus was practical: Understanding what’s happening day to day in a participant’s life and building support around that. Beyond writing plans, our work has always focused on real impact.
We’ve been delivering Positive Behaviour Support (PBS) since it was first introduced under the NDIS. That experience has given us a deep understanding of what quality PBS looks like, and how to deliver it consistently for participants, families and referrers.
As we expanded to more states, we made a key decision, that PBS wouldn’t be one of many services, it would be our sole focus. Because when PBS is everything you do, you do it with more depth, more consistency, and more care.
Today, our team is made up of people who are deeply committed to the work, including many who have lived experience with the NDIS, either personally or through their families. That perspective shapes how we show up, how we listen, and how we design support.
For participants, families and support people, it means support that is clear, practical, and built around their life.
For referrers, it means a team that you can trust to deliver sector-leading support.
For clinicians, it means being part of a team that is focused, supported, and serious about doing PBS properly.
